8th graders will be going on a college visit on March 8th and we would love to have parents/guardians as chaperones. By submitting this online form, the parent/guardian agrees the student listed may attend this field trip and ride DMPS bus transportation to and from Central College in Pella, IA. Parent/Guardian also agrees that should the student have their picture taken, the student's image may be used by Harding, IHDF Des Moines, or Central College for the purpose of promoting our partnership and visit experience.
The parent also agrees that DMPS, I Have a Dream Foundation Des Moines, and Central College, is not responsible for lost or stolen items and shall not be held liable for any incident arising from participation in the trip. Parent/Guardian agrees to allow the adults conducting the trip to make decisions of safety and well-being in case the student listed needs medical attention while in their care.
