Community Mental Health Transformation (CMHT) is underway in Gloucestershire, with the Forest of Dean the first area of focus.
Please come and join us on January 31st to learn about our proposed new Community Mental Health Model working across Primary Care, Voluntary, Community and Social Enterprise Services (VCSE), and Community Mental Health Services with Service Users, Carers and Experts by Experience.
We will have a greater focus on joined-up and seamless care pathways and services and a more personalised approach to outcomes, care planning, recovery and support.
We've made some great progress already but there is still much more to do. Come along and find out more with a briefing from the project team and get involved by sharing your thoughts and feedback.
Everyone with an interest in community mental health services is welcome - service users, carers, experts by experience, professionals and partners.
The event will start at 2pm but please arrive from 1.30pm onwards for signing in, refreshments and networking. The event is being held in the Edward Tauber Suite. Free parking is available.
