Welcome to the Somerset Council SEND Professional Portal

The SEND Professional Portal is used by Somerset Council's Statutory SEND Team as a resource to gather the key information needed around an Education, Health and Care (EHC) Assessment or an EHC Plan Annual Review.

It allows education settings to submit their SEND Support Reviews and Requests for EHC Assessment, as well as Appendix Bs for students who are currently under assessment and Annual Reviews of students who already have EHC Plans.

It also allows professional contributors to submit Appendix Cs for Medical Advice, Appendix Ds for Educational Psychologist Advice, Appendix Es for Early Help and Social Care Advice and Appendix Fs for Children and Young People's Therapy Service as part of the EHC Assessment process.

If you don't already have an account for the SEND Professional Portal, you will need to register using the link on the top left hand side of this page, and you will need to choose an e-mail address with which to register.

We would recommend that you register with a generic e-mail address to this site (ie senco@), rather than a personal e-mail address (ie jsmith@) as this will mean that more than one person with your Education Setting or Service will be able to log in on an account. It will also ensure a seamless handover if there are ever any changes of staff in your Education Setting or Service.

If you require user guidance or need to download any blank/template documents which can be made as part of a submission, please visit the Professional Choices SEND Professional Portal webpage, which is dedicated to this.

If there are students who are under EHC Assessment and do not appear on your Portal account, and your Education Setting or Service is involved, please e-mail sendassessmenthub@somerset.gov.uk and ask to be added onto them as an Authenticated Portal User, ensuring that you send on details of the student or students in question.

If there are students who have an EHC Plan and do not appear on your Portal account, and your Education Setting or Service is involved, please e-mail sendreviewinghub@somerset.gov.uk and ask to be added onto them as an Authenticated Portal User, ensuring that you send on details of the student or students in question.

Once you have registered for an account on the Halo Self Service Helpdesk, you can log any technical issues or queries regarding the SEND Professional Portal, via this online form or by calling the Schools IT Helpdesk on 01823 355090.
